Application Guidelines
Follow these guidelines for completing the application.
Features of Successful Proposals
1. Allowable content
- Limit your proposal to include only information requested.
- Only proposals that adhere to page and word limits will be considered.
- Do not include your own cover letter, videos, brochures, or additional photos.
2. Allowable pages and formatting

Proposals must be presented in the exact order outlined in the application content outline , conforming to page limits as noted for each section and as dictated by the forms provided.

All pages in the Project Narrative must be numbered sequentially and include the school’s name on each page as a footer.

Proposals must use 1 inch margins, a minimum 11 point font, single spacing, and 8 ½ x 11 inch paper.
3. Number of Submissions and Delivery
Provide one, ORIGINAL HARD COPY single-sided complete proposal submission, marked ORIGINAL and one electronic version submitted by email to:

4. Deadline
The complete proposal submission must be received by 5:00 pm on Friday, August 15, 2008.
FUNDING AGREEMENT
Successful applicants will be required to execute a funding agreement with the Medical College of Wisconsin following the notification of award.
INTELLECTUAL PROPERTY AGREEMENT
An Intellectual Property Agreement may be required for inventions, discoveries or copyrightable material developed as a result of a project.
PUBLICITY
All funded schools are asked to provide copies of any press releases, articles or other project publicity to Judi Rehm, project manager. Publicity should identify the Medical School Palliative Care Education Project, coordinated by the Medical College of Wisconsin as the project coordinator, with funding provided by the Robert Wood Johnson Foundation.
